Health Promotion Specialists (HPS) have a role in the area of health promotion, disease prevention and chronic disease management within the family health team setting. Health promoters work with an interdisciplinary team to facilitate various programs. The goal is a proactive, patient-centred approach that empowers patients to take greater responsibility for their own health.
